School District IEPs

To request an IEP assessment for music therapy, please print out the three documents linked to your right. The checklist should be filled out by a parent or caregiver and any other person familiar with how the child responds to music, such as a teacher, school aide, speech therapist, OT or PT. The forms can then be presented to the IEP team.
